Please read about the survey on your handout then click on the link above. You will go to a new window where you can enter your personal survey access code. This starts with the letter 'S' and is then followed by three sets of five numbers each.
If the survey does not open, make sure that you are not blocking any pop-ups or click on the link under More... to the right of this screen.
Please be assured that your answers are completely confidential - contact us if you need to know more.
You should have a version 6 or higher internet browser (Firefox or IE6) and you should have Java Script and cookies enabled - talk to your IT support if you are unsure about this.
Answer the questions by clicking in the relevant boxes. If you keep up a comfortable pace, you should finish in 10-15 minutes. It is best to answer the survey in one sitting but if you need to you can save your answers and come back to complete it a little later.
When you have completed your survey, you must either 'Finish' or 'Save' your survey. Simply closing the browser window will not send us your answers.
You will be prompted to close the browser window when you have finished.
If you simply close the browser, or let the survey time out, you will lose all your answers.
